Tournament Reimbursement Form

Use this form to request your team's tournament reimbursement funds. Reimbursement checks will be mailed after June 30th of the playing year. Teams must complete 10 volunteer hours (attain 20 credits) prior to submitting for reimbursement.

Your volunteers must be submitted using the volunteer registration form at right. Each volunteer will register themselves. Volunteer hours will be tracked and matched with your tournament reimbursement request in order to determine eligibility for a refund.

Enter the name and cost of tournaments that your team is participating in. Reimbursement will be made for the actual amount paid for tournaments, not to exceed the maximum allowed.
